Billing for pain management practices
Interventional procedures, frequency limits, prior authorizations and drug units all decide whether a pain claim pays. We keep each one in order so your physicians can focus on patients.
Where pain management practices lose money
Prior authorizations and frequency limits
Injections, ablations and stimulator trials often need authorization and have payer limits on how often they can repeat. We check both before the date of service.
Medical-necessity documentation
Payer and Medicare coverage policies spell out what the note must show, such as diagnostic blocks before radiofrequency ablation. We flag gaps before claims go out.
Bilateral, multi-level and imaging rules
Levels, laterality, add-on codes and bundled imaging guidance are easy to get wrong. We code them to each payer's rules.
Workers' comp and personal injury
Different payers, different forms and slower payment. We follow these claims to payment.

How it worksFAQ
Pain management billing questions
Do epidural steroid injections need prior authorization?
Often, yes. Many commercial and Medicare Advantage plans require prior authorization for epidural steroid injections, facet procedures and radiofrequency ablation, and many limit how often they can be repeated. We check both before the date of service.
What documentation does radiofrequency ablation need?
Coverage policies usually require chronic pain that has not responded to conservative care and a documented positive response to diagnostic medial branch blocks before ablation. Medicare's local coverage policies for facet joint procedures set specific requirements, and many commercial payers follow similar rules.
Is fluoroscopic guidance billed separately from the injection?
It depends on the code. For many spinal injection codes, imaging guidance is included in the procedure and cannot be billed on its own; for others it is separate. Billing bundled guidance causes denials, and missing separate guidance leaves money unpaid.
Do you bill workers' comp and personal injury cases?
Yes. These claims use different payers, forms and rules from health insurance, and they pay more slowly. We follow them through to payment.
